How To Fill This Form
Step 1: Applicant Name
Shri/Smt./Kumari के बाद applicant का पूरा नाम दर्ज करें।
Step 2: Father/Wife/Daughter Details
Applicant के संबंध के अनुसार पिता/पति/अन्य संबंधित व्यक्ति का नाम भरें।
Step 3: Residential Address
Applicant का पूरा residential address दर्ज करें।
Step 4: Enrollment Date
Accredited Driving Training Center में applicant के enrollment की तारीख भरें।
Step 5: Form 14 Register Number
Training center के Form 14 Register में applicant का serial number दर्ज करें।
Step 6: Training Course
Applicant ने कौन सा driving training course पूरा किया है, उसका विवरण दर्ज करें।
Step 7: Vehicle Class
Form में तीन vehicle classes के लिए स्थान दिया गया है:
Vehicle Class 1
Vehicle Class 2
Vehicle Class 3
Applicant द्वारा पूरी की गई applicable vehicle class की जानकारी यहां दर्ज की जाती है।
Step 8: Aadhaar Number
Applicant का Aadhaar Number दर्ज करें। Form में इसे driving test exemption के लिए applicable बताया गया है।
Step 9: Training Period
Training की From Date और To Date दर्ज करें।
Step 10: Training Completion
Training center यह certify करता है कि applicant ने prescribed syllabus के अनुसार training course satisfactorily पूरा किया है।
Step 11: Physical Fitness & Responsibility
Certificate जारी करने वाला अधिकारी यह प्रमाणित करता है कि वह applicant की physical fitness और sense of responsibility से संतुष्ट है।
Step 12: Authorized Person Details
अंत में authorized person का:

Name
Designation
दर्ज किया जाता है। इसके साथ accredited driving training center का नाम, address, licence number और licence issue date भी दिया जाता है।
